Liability Waiver & Release

In consideration of being permitted to participate in any and all activities, classes, and programs offered by Silverback MMA & Fitness (“the Gym”) located at 937 Manor Blvd, San Leandro, CA 94579, I hereby acknowledge, agree, and represent that I understand the nature of martial arts, fitness, and combat sports training, and that I am qualified, in good health, and in proper physical condition to participate in such activities.

I acknowledge that the activities involved in martial arts and fitness training involve risks of injury including, but not limited to, physical injury, broken bones, sprains, strains, bruising, concussion, and in extreme cases, death. I voluntarily agree to assume all risks associated with participation.

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless Silverback MMA & Fitness, its owners, officers, coaches, instructors, employees, volunteers, and agents from any and all claims, damages, losses, or expenses arising out of my participation in any activities at the Gym.

I understand that this waiver is intended to be as broad and inclusive as permitted by California law, and if any portion is held invalid, the remaining portions shall continue in full legal force and effect.
